PCHD coming to DMS
Perry County Health Department is coming to Du Quoin Middle School to give the Tdap immunization that is needed for all children entering into the 9th grade. All students entering into high school are required to receive a booster Td/Tdap immunization. The PCHD will offer a special immunization clinic for all eighth grade students entering high school this fall. The Clinic will be given on March 25, 2010.
Due to special grant funding, the fee for this immunization will be paid for by the grant or billed to Illinois Medical Card when applicable. THIS SHOT WILL BE GIVEN TO YOUR CHILD AT NO CHARGE!
If you wish for your child to receive this immunization at school please watch for letter home to parents, fill out the form and send it to the Nursing Office. Your child will not be allowed to attend High School unless immunizations are up to date.
